Greene County restaurant inspections for June 3-9

Food establishments in Greene County are inspected one to three times per year, depending on the type of food served, the population served, the difficulty level of food preparation, and past history.

Issues found during inspections fall into two categories: priority and non-priority. Priority violations are issues that can have a direct impact on the safety of the food and can lead to a restaurant getting shut down if they persist. Non-priority violations are usually centered on things like sanitation or building maintenance.

Here is information from the Springfield-Greene County Health Department regarding restaurant inspections over the past seven days:

21-Mo’ Mushrooms, 2144 E. Republic St. 06/04/2022 – Farmer’s Market Inspection; Result: Active. Priority Violations Found: 0. Nonpriority Violations Found: 0.

Abby’s Acai Company, 2144 E. Republic St. 06/04/2022 – Routine Inspection; Result: Active. Priority Violations Found: 2. Observed bleach sanitizer bottle inappropriately labelled. Person in charge peeled off old label and added appropriate label. Observed substantial leak from plumbing, so person in charge had shut off water to sinks. Fix plumbing to be in good repair. Nonpriority Violations Found: 0. 

Am Donuts And Coffee, 1501 E. Pythian St. 06/04/2022 – Routine Inspection; Result: Permitted with Reinspection. Priority Violations Found: 1. Observed half and half in cooler at 48F, and air temperature of 48F. Person in charge stated it had been in there all morning. Adjust/repair/replace unit to hold 41F or below. Placed remaining half and half on time as a control and educated staff on process of time as a control.  Nonpriority Violations Found: 0. 

Chameleon Cuisine, Truck. 06/04/2022 – Routine Inspection; Result: Active. No violations. Priority Violations Found: 0. Nonpriority Violations Found: 0.

Corner 21 Chinese Cuisine, 1369 E. Sunshine St. 06/08/2022 – Routine Inspection; Result: Permitted with Reinspection. Priority Violations Found: 1. Observed:  Dish washing occurring with no sanitizer made in the 3-vat sink area.  Dish washing was stopped and education given on proper dish washing procedures. Nonpriority Violations Found: 0.

Culver’s Of Springfield, Sunshine. 06/08/2022 – Routine Inspection; Result: Active. No violations sited during the inspection. Priority Violations Found: 0. Nonpriority Violations Found: 0. 

Dollar Tree #7439, 2759 W. Republic St. 06/10/2022 – Routine Inspection; Result: Active. Priority Violations Found: 0. Nonpriority Violations Found: 1. Observed gap at middle of receiving doors at the bottom. 

Dugout, 1218 E. Trafficway St. 06/08/2022 – Routine Inspection; Result: Active. No violations cited during inspection. Priority Violations Found: 0. Nonpriority Violations Found: 0.

Echelon Coffee, 2407 N. Glenstone Ave. 06/06/2022 – Routine Inspection; Result: Active. Priority Violations Found: 2. Observed: Can opener blade found not clean. Education given and corrected during the inspection. Observed: Dented can of salsa found on storage shelf. Education given and corrected during the inspection. Nonpriority Violations Found: 3. Observed: Bag of flour found stored directly on the floor. Observed: No test kit to check the concentration of sanitizer. Observed: Outside of food containers found with sticker residue from old date labeling.

El Sombrero, 1529 W. Battlefield Rd. 06/08/2022 – Routine Inspection; Result: Active. Priority Violations Found: 1. Observed shrimp stored in bottom low boy drawer cooler beneath chicken in top drawer. Corrected on-site. Nonpriority Violations Found: 0. 

Exit 88 Diner, 225 E. Evergreen St Strafford. 06/06/2022 – Routine Inspection; Result: Active. Priority Violations Found: 2. Observed dishwashing machine not dispensing correct concentration of sanitizer. PIC called maintenance and while waiting for repair using 3-comp sink with proper sanitizer concentration to clean dishes. Dishwashing machine sanitizer concentration issue was corrected by maintenance worker on site. Observed multiple lasagna squares not properly date labeled. Education given and corrected on site by staff labeling the squares with the day of preparation. Nonpriority Violations Found: 1. Observed through out the back half of the kitchen few live flies. 

Grand Mart, 1310 W. Grand St. 06/06/2022 – Routine Inspection; Result: Active. Wally was person in charge at time of inspection. Outdated products being held in home style refrigerator to be returned for credit. Priority Violations Found: 0. Nonpriority Violations Found: 4. Observed: The ice bagged on premises did not have a phone number or name and address of business where ice was manufactured. Observed: A few ceiling tiles in retail store were missing. Observed: Back exterior door by ice machine is not self-closing. Observed: Open bag of coffee filters were being stored under drain lines for front hand sink.

Jose Loco’s Steak & Grill, 300 E. Battlefield Rd. 06/09/2022 – Routine Inspection; Result: Active. Priority Violations Found: 0. Nonpriority Violations Found: 1. Observed open employee drink in food preparation area.

Kum & Go #486, 125 S. State Highway 125 Strafford. 06/06/2022 – Routine Inspection; Result: Active. Priority Violations Found: 0. Nonpriority Violations Found: 2. Observed tiles in back room missing or in need of repair and a ceiling tile in front of the house with water damage. Required physical facilities are kept in good repair. Observed missing wall coving along side few walls in the back room and a hole in the wall in the right corner of the front wall. Required to keep physical facilities in good repair.

Kum & Go #499, 1894 S. State Highway 125, Rogersville. 06/06/2022 – Ownership Change; Result: Approved. Priority Violations Found: 1. Observed commercially made burrito at 127F, and commercially made sausage sandwich at 131F. Person in charge stated they had started the reheat process in the hot case over 2 hours ago. Discussed microwaving product and checking to make sure appropriate temperature is reached within 2 hours. Person in charge voluntarily discarded foods. Nonpriority Violations Found: 3. Observed franks in freezer at 45F. Freezer did not appear to be running, no cold air was blowing. PIC stated it was working when he came in that morning and he moved foods to walk-in to cool. He said he will defrost to try to fix issue. Adjust/repair/replace unit to maintain foods frozen. Observed deteriorated grout and bulid-up of dirt/debris behind coke cooler and 1 door True cooler. Fix surfaces to be easily cleanable and maintain clean. Facility did not have test kit available for sanitizer solution. Provide test kit for sanitizer solution.

Love’s Travel Stop & Country Stores, Inc, 400 N. State Highway 125 Strafford. 06/06/2022 – Routine Inspection; Result: Permitted with Reinspection. Priority Violations Found: 1. Observed mold build-up in the ice dispensing tube. Required equipment food contact surfaces shall be clean to sight and touch. Re-inspection will be conducted. Nonpriority Violations Found: 1. Observed packs of water stored on the floor in the back storage room and the hallway leading to the restrooms.

Love’s Travel Stop Hardees, 400 N. State Highway 125 Strafford. 06/06/2022 – Routine Inspection; Result: Active. No violations were cited during the inspection. Priority Violations Found: 0. Nonpriority Violations Found: 0.

Mcstop Of The Ozarks, 333 E. Evergreen St., Strafford. 06/06/2022 – Routine Inspection; Result: Active. Priority Violations Found: 0. Nonpriority Violations Found: 1. Observed food handling employees wearing visors. Visors are not considered a form of effective hair restraint. Food handling staff replaced their visors with proper hair restraints.

Momo Sushi And Grill, 2767 W. Republic St. 06/07/2022 – Routine Inspection; Result: Permitted with Reinspection. Priority Violations Found: 1. Observed sushi coolers at bar holding fish at 47-55F. All fish voluntarily discarded. Nonpriority Violations Found: 3. Observed several flies in kitchen area around food contact surfaces of equipment/ware. Observed build up of grease on vent hood filters, frame and ancillaries. Increase cleaning frequency to prevent build up. Observed no time control policy in place for the sushi rice. Example policy provided with inspection report. Time noted on sushi rice.

Mr. Goodcents Subs & Pastas, 2445 N. Kansas Expy. 06/03/2022 – Routine Inspection; Result: Permitted with Reinspection. Genny was person in charge at time of inspection. A follow up visit will be made on or after 6-24-2022 to check for compliance. Priority Violations Found: 0. Nonpriority Violations Found: 4. Observed: The hot water at front hand sink was shut off, due to a leak. Observed: Moderate buildup on floor under equipment. Also floor beside ice machine was not clean. Observed: The inside of ice machine had black mold and debris on the deflector shield. Observed: One thermometer in upright refrigerator was inaccurate – alcohol had separated.

Pa Chan’s Asian Kitchen, 3245 W. Republic St. 06/06/2022 – Re-Inspection; Result: Active. Priority items corrected. Priority Violations Found: 0. Nonpriority Violations Found: 0.

Papi’s Cocina & Cantina, 808 S. Illinois Ave., Republic. 06/07/2022 – Compliance Check 1; Result: In Compliance. On site for compliance check to verify walk-in cooler is holding 41F or below or no potentially hazardous foods are being stored in it. Walk-in in storage area(walk-in #2) holding 59F. Walk-in in dish pit area(walk-in #1) holding 41F. Cold holding violation has been corrected. All potentially hazardous foods are being stored in walk-in #1. Walk-in #1 sufficient to support menu. If issues with cooling or cold holding occur refrigeration space may be reevaluated. Priority Violations Found: 0.  Nonpriority Violations Found: 0.

Raising Canes Chicken Fingers #513, 321 W. Sunshine St. 06/08/2022 – Routine Inspection; Result: Active. Ethan was person in charge at time of inspection. Discussed where employees are allowed to eat. Priority Violations Found: 0. Nonpriority Violations Found: 1. Observed: Some boxes of frozen food stored on floor of walk-in freezer, due to inadequate freezer space.

Schultz & Dooley’s, 2210 W. Chesterfield Blvd. 06/08/2022 – Routine Inspection; Result: Active. Priority Violations Found: 0. Nonpriority Violations Found: 2. Observed kitchen hand sink drain clogged, the water is draining very slowly. Observed lid not on dumpster due to trash being stacked to high and recycled can goods on ground un bags. Schedule more frequent pick ups with service or obtain a larger trash can and obtain a recycle bin for items stored on ground. (This will help prevent fly issues with summer here)

Sno Biz #3, 1707 W. Battlefield Rd. 06/09/2022 – Routine Inspection; Result: Permitted with Reinspection. Kyler was person in charge at time of inspection. I was informed that they use Bolivar City Water and freeze it in Bolivar, then distribute to their mobile units. Permit not issued, due to plumbing issues. Priority Violations Found: 0. Nonpriority Violations Found: 2. Observed: Faucets at two sinks were not operational, due to leaks. Observed: The floor in mobile was sticky and needs cleaned better.

Taco Bell #018190, 610 E.Us Highway 60 East, Republic. 06/06/2022 – Routine Inspection; Result: Active. Discussed service dog and animal restrictions in a food establishments Priority Violations Found: 0. Nonpriority Violations Found: 2. OBSERVED:  HOT WATER BELOW 100F IN BOTH BATHROOMS.OBSERVED:  NO SOAP AT MEN’S BATHROOM HAND SINK.

The Big Slice, 1454 E. Sunshine St. 06/08/2022 – Routine Inspection; Result: Active. No violations cited at time of inspection. Priority Violations Found: 0.  Nonpriority Violations Found: 0. 

Tortilleria Perches, LLC, 1601 W. Sunshine St. 06/09/2022 – Routine Inspection; Result: Active. Priority Violations Found: 0. Nonpriority Violations Found: 1. Observed ground beef cooling in closed containers in quad door cooler. Use an approved method such as walk-in freezer or walk-in cooler, in shallow uncovered container. Corrected by placing in walk-in freezer as cooling was still within 2 hours of beginning of process.

White Oak Station #63, 310 S. Scenic Ave. 06/08/2022 – Routine Inspection; Result: Permitted with Reinspection. Priority Violations Found: 1. OBSERVED:  SEVERAL UNITS OF MEDICINES FOR SALE PAST EXPIRATION DATE. Nonpriority Violations Found: 0.

Wicked Wok, 4852 S. State Highway Ff Battlefield. 06/07/2022 – Re-Inspection; Result: Active. Priority and core items corrected. Priority Violations Found: 0. Nonpriority Violations Found: 0.

Wicked Wok, 4852 S. State Highway Ff Battlefield. 06/06/2022 – Routine Inspection; Result: Active. Priority Violations Found: 2. Observed build up of grease and debris on working chicken containers and steam table containers. Owner stated he would buy new containers this evening. Observed PHFs in cooler made week prior without seven day use by noted on container or log. Corrected on-site. Nonpriority Violations Found: 2. Observed no cooking thermometer with thin probe available for cook’s use. Observed leaking kitchen hand sink. Sink needs to be working so hands can be washed at sink. The mop sink is immediately adjacent and at suitable level to use for hand washing today. Sink needs to be repaired by 6/7/2022.
